ADOPTION
If the child is of Indian Heritage, you must request the
Indian Child Welfare Act (ICWA) Packet #P2
Forms can be printed and completed by hand in black ink or typewriter, or
filled in on-line and printed on your own printer using any PDF viewer
program such as Adobe Reader.
Filing Fee: $20 for each person being adopted.
Fee Waiver available upon request.
